Printable Mail Order Form

We have created two versions of our Printable Mail Order Form. Please click on the appropriate browser version that you are using to get to the correct Mail Order Form.

Printing Instructions: Setting your Page Set-up by clicking on your Browsers "File" then select "Page Set-Up" Change Top and Bottom margins to "0", uncheck all Header and Footer content. By doing this you will be able to print the order form on One Page. Click on "Print This Page" at the top left hand corner of the Mail Order Form. Then make sure to set your printer to print 1 of 1.

Shipping Calculation: Please see our Service Policy for help in determining your shipping charges.

Sales Tax: 5% Sales Tax only applicable to residence of Illinois. When ship to address differs from Illinois Billing Address, sales tax remains applicable.

When paying with a Personal Check please add up to 7 business days to domestic delivery date of 2 to 4 weeks. This is to allow for check to be processed by the bank system.

When paying with a Money Order expect delivery within 4 weeks of mail order form receipt.

Mail Order Customers will be notified by email upon receipt of Mail Order Form and payment.

Priority Mail can be requested. Please check appropriate box. Additional charges will be requested prior to order shipment. We cannot guarantee 3 day delivery for orders shipped outside the contiguous United States or during holidays and peak season for the postal service.
